RCA: PIC shall have form saved for employees to reference. Forms are to be completed annually. Upon request, the operator shall provide evidence of inspection and/or correction of any deficiency

Observed burn holes on linens in rooms 423 (mattress cover),419 (mattress cover), 425 (fitted sheet and flat sheet). Observed staining in rooms 423 (mattress), and 425 (mattress pad). Observed debris on top of mattress in room 425.

RCA: PIC shall remove and discard linens with burn-holes and treat all stains above. All linens shall be free of stains and debris and shall be maintained in good repair

Observed hot water to exceed the maximum 120F in every room inspected. Hot water temperatures ranged from 123F-127F. Observed annual gas fire equipment inspection wasn't accessible upon request. Rule: All gas-fired equipment shall be inspected at least annually by a qualified licensed contractor or local fire authority, if available. Points to be inspected are proper construction and installation, malfunctions and adjustments of controls and burners, faulty heat exchangers and vent obstructions. Any defects found on inspection must be corrected by a qualified heating contractor prior to use of the equipment. Upon request, the operator shall provide evidence of inspection and/or correction of any deficiency

Observed staining on pillows in rooms 310,312,322. Rule: All furniture, draperies, appliances, carpets, and other accessories, in the tourist accommodation must be maintained in good repair, clean, and free of vermin

Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin. Each bed shall be provided, as a minimum, with two sheets and one pillow and pillowcase. After each occupancy and upon the request of the occupant, sheets and pillowcases shall be changed with fresh laundered linens. During occupancy, linens shall be changed at a minimum frequency of not less than weekly
