Observed vents in all bathrooms to have a heavy built-up of lent making it difficult to ventilate the bathroom.

RCA: Told PIC to ensure that all vents are clean. In existing tourist accommodations permitted prior to the adoption of this rule, the current ventilation must prevent odor, mold, mildew, and moisture

Observed staining to pillow cases, mattress, and mattress pad in rooms 219,226,309, 314.

RCA: Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin. After each occupancy and upon the request of the occupant, sheets and pillowcases shall be changed with fresh laundered linens

Observed that facility did not have gas fire inspection this time.

Observed heavy staining on the mattress in room 201. Observed light staining on mattress in room 206, 230, and 309. Observed underwear under the mattress in room 224. Observed shower curtain mounts not securely attached to the wall in rooms 106, 116, 201, 309, and 313. Observed toilet paper holder not securely attached to the wall in 206, 230, and 313. Observed caulking in shower to be discolored in room 232 and coming off in room 224. Observed missing outlet cover in room 313. Observed chip on bathroom counter in room 313. RCA: Toilet rooms and fixtures must be smooth and nonabsorbent and shall be kept clean and in good repair. Walls, floors, and ceilings shall be constructed of easily cleanable nonabsorbent materials and shall be kept clean and in good repair

Observed hot water to exceed 120F (122F - 135F).

RCA: Hot water in all guest rooms shall not exceed 120F

Observed the front and top of the outside of the fridge to be peeling and observed a rust like substance in the freezer part of the fridge in room 118. Observed food debris to be left in the microwave and observed trash on the floor by fridge in room 221. Observed a major buildup of cardboard and food debris stuck to the ice in the freezer portion of the fridge and the racks to be peeling in the fridge in room 207.

RCA: All furniture, draperies, appliances, carpets, and other accessories, in the tourist accommodation, whether the property of the tourist accommodation owner or not, shall be considered the property of and furnished by the tourist accommodation for purposes of enforcement of this Chapter, and must be maintained in good repair, clean, and free of vermin

Observed major staining on the mattress and brown blanket, as well as holes in the top comforter in room 121. Observed staining on the mattress cover and food debris on the sheets in room 221. Observed a major yellow stain on the mattress cover in room 207. Observed dirt/debris on the mattress cover and holes in the comforter in room 213.

RCA: Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin

Observed a strand of hair on the toilet in room 213. Observed a big crack on the shower wall in room 207. Observed chipping on the tub in room 221. Observed the coping between the tub and the wall to be peeling and cracked in room 118.

RCA: Toilet rooms and fixtures must be smooth and nonabsorbent and shall be kept clean and in good repair. Walls, floors, and ceilings shall be constructed of easily cleanable nonabsorbent materials and shall be kept clean and in good repair

Observed a live roach in room 125.

RCA: Effective and appropriate measures shall be taken to eliminate the presence of rodents and flies, roaches, bed bugs, and other insects on the premises

Observed build-up of a black mold-like substance on baffles of 1st and 2nd floor ice machines.

RCA: PIC placed not working signs on ice machines until they can be cleaned and sanitized
