3-1C - food in good condition, safe, and unadulterated

Found mold-like growth on snow peas. Found shrimp with an offensive odor. COS: Charlton EH had CFSM discard the peas and shrimp.
1-2A - pic present, demonstrates knowledge, performs duties

PIC not performing duties as evidenced by multiple critical violations.
4-2A - food stored covered
Regulation: 511-6-1.04(4)(c)1(iv) - packaged & unpackaged food, food stored covered(c)
Observed a bulk container of broccoli in the WIC not covered while stored. COS: Discarded, Charlton EH told CFSM that all foods not actively cooling or being worked must be covered.
4-2B - food-contact surfaces: cleaned & sanitized
Regulation: 511-6-1.05(7)(b) - food contact surfaces and utensils - cleaning frequency (p, c)
Observed food residue/grease build-up on pans/containers. CA: Charlton EH instructed CFSM to clean pans with build-up on them or to replace them.
6-2 - proper date marking and disposition
Regulation: 511-6-1.04(6)(h) - ready-to-eat time/temperature control for safety food, disposition (p)
Found TCS foods out of date in the WIC: shrimp (11-3), pork (11-10), and noodles (10-27). The shrimp had a very strong odor to it, but CFSM stated the date was supposed to be 11-17. COS: All foods discarded and Charlton EH instructed CFSM to properly date mark TCS foods as the CFSM stated they just put a new sticker on the containers when adding new product instead of removing the old stickers.
16C - sewage and waste water properly disposed
Regulation: 511-6-1.06(4)(d),(e) - grease trap; conveying sewage (c, p)
Handwash sink is not draining properly, sink filled up to the top of the basin when inspector used sink at the start of this inspection. The water level did not go down during the duration of this inspection. CA: Charlton EH instructed CFSM to have the sink plumbing serviced to ensure it drains properly.
15A - food and nonfood-contact surfaces cleanable, properly designed, constructed, and used
Regulation: 511-6-1.05(2)(a) - equipment and utensils, constructed of durable materials (c)
Found two (2) broken plastic containers containing food. COS: Charlton EH ordered CFSM to discard the containers and the food inside to prevent the serving of food with a physical contaminant. Charlton EH again told CFSM to obtain food grade bins to store food.
15C - nonfood-contact surfaces clean
Regulation: 511-6-1.05(7)(a)2,3 - equipment, food/nonfood-contact surfaces, and utensils, food-contact surfaces of cooking equipment & nonfood-contact surfaces free of accumulations (c)
Observed food residue and/or grease build-up around or on cooking equipment in the kitchen. CA: Equipment shall be cleaned at a frequency to prevent the accumulation of build-up or other deposits.
17C - physical facilities installed, maintained, and clean
Regulation: 511-6-1.07(5)(a),(b) - good repair, physical facilities maintained; cleaning, frequency & restrictions, cleaned often enough to keep them clean (c)
Observed large grease spills on the floor under/behind the woks and fryer, food residue everywhere, and a large patch of mold-like growth on the wall behind one of the woks. CA: Charlton EH instructed CFSM to clean the kitchen and to clean it more frequently to prevent the accumulation of build-up.
