Inconsistent date marking. CA: Charlton EH instructed PIC to ensure all foods are properly date marked for prep or use by date.
10D - food properly labeled; original container

Observed foods out of original container not labeled with common name of food. CA: Charlton EH instructed PIC to label all foods out of original containers.
12A - contamination prevented during food preparation, storage, display
Regulation: 511-6-1.04(4)(q) - food storage (c)
Foods not being stored in freezers according to cook temperatures when opened/removed from original packaging. Observed some foods (meat) being stored in used bread bags or in nothing at all: bare, cooked turkey legs resting on top of boxes in freezer; raw hamburger patties fallen out of bag sitting on top of French fry bags. CA: Charlton EH reviewed with PIC proper food storage based on cook temperatures and ordered that foods be stored properly in food grade containers. Partially COS: turkey legs were discarded and hamburger patties discarded.
18 - insects, rodents, and animals not present

Observed numerous live flies flying around in the kitchen, dry storage, and outside near the smoker where the grease has spilled/accumulated on the ground. Observed numerous dead roaches in areas throughout the FSE (dry storage, kitchen, bar). CA: Charlton EH instructed PIC to seal gaps in the doors/building and to clean up the outside to prevent harborage and attraction of pests.
16C - sewage and waste water properly disposed

Observed condensate dripping inside reach-in-cooler into a pan. Observed grease collection in plastic buckets by smoker/spilling onto ground. CA: Charlton EH told PIC to repair or replace RIC and catch/dispose of grease properly.
17A - toilet facilities: properly constructed, supplied, cleaned
Regulation: 511-6-1.07(5)(i) - closing toilet room doors (c)
The bathroom in the kitchen still does not have a self-closing door. CA: Charlton EH told PIC again that this door must be self-closing.
17C - physical facilities installed, maintained, and clean
Regulation: 511-6-1.07(5)(a),(b) - good repair, physical facilities maintained; cleaning, frequency & restrictions, cleaned often enough to keep them clean (c)
Observed grease spillage on the floor in the kitchen, food residue on the floor and on equipment, mold-like growth in certain areas (on a piece of tape on a box with lettuce in the reach-in-cooler, dry storage), gaps in the doors/door sills, exposed wall sockets/gaps in the walls. CA: Charlton EH instructed PIC that damaged areas need to be repaired, the FSE needs a deep clean, and to be cleaned at a frequency to prevent accumulations.
17D - adequate ventilation and lighting; designated areas used
Regulation: 511-6-1.07(2)(i) - light bulbs, protective shielding (c)
No light shields on lights in kitchen or food prep areas. Repeat. CA: Charlton EH instructed FSE to put shields on lights to prevent contamination of food if lightbulbs bust. Inadequate lighting in RIC and in dry storage. CA: Charlton EH instructed PIC that these areas are required to have adequate lighting.
