Observed bare cement floor and raw wood in dry storage area where raw animal food is stored.
CA: Floor must be sealed and raw wood painted to make it smooth, durable, and non-porous.
15B - warewashing facilities: installed, maintained, used; test strips

No chemical test kit provided when using chemical sanitizer at three-compartment sink/ware washing machine.
CA: PIC stated test strips are ordered but have not been received yet.

Observed build-up of grease on exhaust hood and grease spills on flooring under fryers, blood pooled in RIC where raw chicken is stored, and food residue from leaking bags in RICs.
CA: Nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues.
17C - physical facilities installed, maintained, and clean
Regulation: 511-6-1.07(5)(a),(b) - good repair, physical facilities maintained; cleaning, frequency & restrictions, cleaned often enough to keep them clean (c)
Observed multiple holes in walls/ceilings, exposed fiberglass insulation, exposed wiring, and old equipment in dry storage.
CA: Damaged walls/ceilings to be patched to prevent pest intrusion, fiberglass insulation to be sealed up in lobby, and wiring to be covered up.
