- Food prepared in a private home or received from a consumer may not be used or offered for human consumption in a food service establishment.

Person In Charge (PIC) is unaware of where the food is sourced from and/or records of food source. Rice, curry, and beef pockets were in to go containers and/or plastic storage bags. CA: Cease to sell of non-commercially sourced foods and start record keeping verifying food is received from commercial deliveries. Please note that it is the responsibility of the PIC to ensure that food service establishment operations are not conducted in a private home and that employees are visibly observing and verifying delivered foods as they are received to determine that they are from approved sources.

Missing allergy awareness and notification - Train employees on allergens that are present and also please note that rule .04(7)(d) requires unpackaged food items containing a major food allergen as an ingredient must have a written notification declaring which major food allergens are contained in that food item.

No proof of a food safety manager on staff. CA: Obtain a food safety manager certification as soon as possible and post for public view. Code: (1) The original CFSM certificate shall be posted in public view in each food service establishment. An additional copy shall be retained on file at the food service establishment at all times, and shall be made available for inspection by the Health Authority. (2) A CFSM certificate which has expired, been revoked or suspended shall not be posted in the food service establishment.
6-2 - proper date marking and disposition

Observed no date labels on multiple cooked/made items. Code requires refrigerated, ready-to-eat, time/temperature control for safety food prepared and packaged by a food processing plant shall be clearly marked, at the time the original container is opened in a food service establishment and if the food is held for more than 24 hours, to indicate the date or day by which the food shall be consumed on the premises, sold, or discarded, when held at a temperature of 41°F (5°C) or below for a maximum of 7 days after the original container is opened.
