At time of inspection, microwaves in most rooms needed to be replaced due to rust. Carpets generally needed vacuuming, especially around fridges and drapes. Table surfaces in most rooms needed wiping down (326,224,119,115). //Advising PIC to thoroughly clean rooms and replace microwaves

At time of inspection, observed lack of anti-slip surface in tubs in rooms 109, 119,223, and 326

At time of inspection, observed exhaust fan in ceiling of bathroom in Room 203 loosened and hanging, popped up through the ceiling. Repair / properly install the exhaust fan and ensure that it is capable of operating properly

At time of inspection, the door to Room 210 was observed to be very difficult to open. The PIC attempted several times to open the door and finally was able to open the door using some force. Doors must be maintained and must be capable of safely opening and closing. Repair door

At time of inspection, observed what appears to be moisture damage around the window at the end of the hall (near the laundry room). Repair areas allowing possible water intrusion and repair/replace materials as necessary

At time of inspection, observed mold-like growth and buildup in numerous rooms (Room 203: mold-like growth on ceiling and on pillowcases; Room 207: mold-like growth on ceiling above the window and on pillowcases; Room 319: mold-like growth on the carpet; Room 116: mold-like growth on ceiling above window; Room 122: mold-like growth on the carpet). Facility appears to have extensive moisture/ventilation issues. All rooms where mold-like growth is observed must be closed to guests and must be assessed and remediated by a licensed professional. The rooms listed above and any other affected rooms must be closed to guests, until moisture/ventilation issues have been resolved and the rooms have been inspected. Recommend consulting a professional regarding the ventilation of the facility, to determine what may be causing the excessive moisture issues.

8/12/2025: Visited facility and spoke with the General Manager (GM). GM confirmed that the affected rooms are blocked as "out-of-order" and will remain closed, until the moisture/ventilation issues have been resolved and the affected rooms have been professionally remediated. GM states that they believe the A/C units are causing the issues, so they are getting quotes to replace the A/C units. GM states he will contact EH Office for inspection, before re-opening the rooms

At time of inspection, observed lack of anti-slip applique or mat in the tub in Room 201. Place anti-slip applique or mat in the tub

At time of inspection, observed stained mattress pad and comforter in Room 112, as well as a significant stain on the pillow case in Room 109. PIC states that new pillows, pillow protectors, and cases have been ordered and will be in place very soon

At time of inspection, observed lack of anti-slip surface in the tub in Room 105. PIC states she will have either non-slip mats or non-slip coating/strips added to tubs
