Observed several employees handling and/or prepping food items with hats on, but hair was hanging out the back. Employees should restrain their hair so that it is not hanging.
16B - plumbing installed; proper backflow devices
Regulation: 511-6-1.06(2)(r) - system maintained in good repair (p, c)
Observed the faucet on the 3-compartment sink loose and dripping water. Also, observed the pipe on the handwashing sink leaking water on the water. Both sinks will need to be repaired.
15A - food and nonfood-contact surfaces cleanable, properly designed, constructed, and used
Regulation: 511-6-1.05(2)(a) - equipment and utensils, constructed of durable materials (c)
Observed the shelves above the salad prep area with a rust on them. Please clean and properly seal the shelves to prevent contamination. Also observed the exterior surface (top) of the ventilation hood system with a black debris on it. Please clean regularly.

Observed water nesting on the floor by the hand washing sinks and by the ice machine. Also, the floor area by the drive-thru window is in need of cleaning.

Observed 1 light out under the ventilation hood system.

Observed an employee salad and drink stored above prepped sauces and cheese products in the walk in cooler. PIC discarded the items (COS).

Observed the HVAC vents and ceiling area around it (utility area) in need of cleaning due to dust debris.
