Wall by the dish washing machine in the back and the prep table to the back left have significant missing portions of wall paper and appear to be crumbling. There missing tiles on the floor bt the dish washing area as well. CA: Floors, walls, and ceilings must be maintained in good repair. Correct within 1 month.

Container of oil on the back prep table uncovered. Container of liquid butter by the oven stored uncovered; CA: All food shall be stored covered to prevent overhead contamination; COS: Covered during inspection.

Wall by the prep table in the back has residue on it. The wall above the prep table that is to the left of the fryer has a build up of residue on it; CA: All physical facilities shall be maintained clean.

Observed unopened bag of banana peppers on the floor in the kitchen behind the shelf they were stored on. CA: Food and single service items shall be stored a minimum of 6 inches off the floor.

Observed employee with fake nails prepparing a pizza with no gloves. Fake nails may not be worn when working with food unless wearing a pair of gloves in good repair. COS: employee washed hands and gloved up.

Multiple pieces of equipment and shelving that are covered in grease and grime. Non food contact surfaces of equipment shall be cleaned at a frequency necessary to prevent the accumulation of dirt and food particles. CA: Clean equipment thoroughly during an end of shift or during a closed day.

Multiple areas of the facility covered in grease and buildups especially in the back corner area near mop sink. Phyical facilites shall be cleaned in a manner and frequency to reduce buildupor dirt and debris. Pull out the back shelving/prep table by the mop sink and clean behind that entire line of equipment. Deep cleaning of floors, walls, and ceiling is NEEDED.

Set of lights near the fryers has no shielding on the lights. Lights that are glass must have a protective shielding in case of breakages. CA: Repair by net inspection.

Multiple shelves with accumulations of grease dripping from them. Non-food contact surfaces shall be cleaned at a frequency to prevent accumulations. CA: Clean shelves within the next 30 days.

Overall facility is dirty and has dust and grease all over ceiling walls, and floors. Floors have multiple area with broke tile . Facility must be clean and kept in good repair. CA: Deep clean facility ASAP and fix broken tile.

Observed two sets of fluorescent lights without shielding. Glass bulbs must have a shielding in case of breakage. CA: Fix lights without shielding or replace with non-glass bulbs.

Observed drain between pizza station and coolers backing up onto floor. Plumbing system shall be maintained in good repair.

Observed pizza boxes and other single use items stored on floor in the small hallway. Single use items must be stored a minimum of 6 inches off floor. COS PIC moved items to proper storage area.

Observed 4 sets of kitchen lights with fluorescent bulbs with no plastic shielding on them. Glass light bulbs must have a protective plastic shielding in case of breakage.
