6-1A - proper cold holding temperatures
Regulation: 511-6-1.04(6)(f) - time/temperature control for safety; cold holding (p)
Observed multiple food items that were 42 degree Fahrenheit in the walk in cooler.
CA: Time/temperature control for safety food shall be maintained at a temperature of 41 Fahrenheit or below to maintain safety of the food.
COS: After further questioning the person in charge, employees left cooler door open during transition which allowed the cooler temperature to climb above 41 Fahrenheit. Informed the PIC to move the items to the back of the walk in cooler, as it was in temperature toward the back. Have maintenance check the seals at the front of the walk-in cooler.
2-2A - management knowledge, responsibilities, reporting
Regulation: 511-6-1.03(2)(o) - person-in-charge duties (pf)
Employee observed not trained on employee health policy for symptoms and illnesses to report out of work with. Could not name symptoms to report nor the 6 food transimissble diseases. CA: Food employees and conditional employees are informed in a verifiable manner of their responsibility to report in accordance with the Chapter, to the person in charge, information about their health and activities as they relate to diseases that are transmissible through food;P COS: Employee was informed about the signs and symptoms she needs to report as soon as those signs are observed. PIC was advised to retrain the employee on the health policy. PIC(Owner) given a copy of the quick decision guide and the employee health policy from state website.
12B - personal cleanliness
Regulation: 511-6-1.03(5)(j) - hair restraints (c)
Observed employee with no hair restraint while preparing food. CA: Employees preparing or handling food shall use effective and clean, disposable or easily cleanable nets or other hair restraints approved by the Health Authority, worn properly to restrain loose hair including beards and mustaches longer than one half inch. COS: Employee made by PIC to put on hair net or hat.
12B - personal cleanliness
Regulation: 511-6-1.03(5)(g) - jewelry (c)
Observed employee wearing bracelets while preparing food on the front food service line. CA: Except for a plain ring such as a wedding band food employees may not wear jewelry including medical information jewelry on their arms and hands while preparing food. COS: Employee made to remove her bracelet and rewash hands before continuing.
16B - plumbing installed; proper backflow devices
Regulation: 511-6-1.06(2)(r) - system maintained in good repair (p, c)
Observed a leak at the back 3 compartment sink. CA: A plumbing system shall be repaired according to law and maintained in good repair. Correct within 1 week.
17B - garbage/refuse properly disposed; facilities maintained
Regulation: 511-6-1.06(5)(o),(p) - using drain plugs; maintaining refuse areas (c)
Observed the outside dumpster with no drain plug present. CA: Drains in receptacles and waste handling units for refuse, recyclables, and returnables shall have drain plugs in place. Request new drain plug from city department and correct within 1 week.
