EHS observed handwash station with empty dispensers.
Handwashing Cleanser, Availability. Each handwashing sink or group of two adjacent handwashing sinks shall be provided with a supply of hand cleaning liquid, powder, or bar soap.
EHS advised PIC to replace cartridge. PIC corrected on site.

EHS observed hole surrounding mop sink fixture due to repairs and pipe replacement at facility.
All physical facilities shall be maintained in good repair.
EHS advised PIC to have plumber replace or plug hole ASAP.

EHS observed facility with CFSM certificate unable to be validated. Violation: At least one employee that has supervisory and management responsibility and the authority to direct and control food preparation and service shall be a certified food safety manager who has shown proficiency of required information through passing a test that is part of an accredited program that conforms to the national standards for organizations that certify individuals. Certified Food Safety Managers must be designated to one food service establishment only and maintain and renew certification in accordance with the requirements of the examination taken. CA: obtain proof of certification or new certification within 60 days.

EHS observed buns having been toasted left on prep table with no protection from environmental contamination. Violation: Food Preparation. During preparation, unpackaged food shall be protected from environmental sources of contamination. CA: EHS instructed PIC to finish process of toasting buns and to finish task prior to starting new task when facility gets busy.

EHS observed PIC preparing ice cream products with no hair restraint. Employees wearing hair restraint of visor however hair is longer than what is able to be contained by visor. Violation: Employees preparing or handling food shall use effective and clean, disposable or easily cleanable nets or other hair restraints approved by the Health Authority, worn properly to restrain loose hair including beards and mustaches longer than one half inch. CA: ensure hair restraints are able to completely and effectively restrain hair.

EHS observed personal item (purse) stored on top of single service boxes used for serving product. Violation: personal items shall be stored in designated areas where contamination of food, equipment, utensils, and single-service items is prevented. CA: PIC moved purse to designated area.
