8 - annual gas fired equipment inspection on file; gas units equipped with automatic safety and/or 100% cut-off pilot
Regulation: 511-6-2-.15(5) - heating and fire safety-inspection
EHS observed no annual gas-fired equipment inspection on file and available for view. The PIC indicated that an inspection was performed relatively recently; however, the PIC indicated that the contractor they use does not store service records.
511-6-2-.15(5) - Heating and Fire Safety-Inspection All gas-fired equipment shall be inspected at least annually by a qualified licensed contractor or local fire authority, if available. Points to be inspected are proper construction and installation, malfunctions and adjustments of controls and burners, faulty heat exchangers and vent obstructions. Any defects found on inspection must be corrected by a qualified heating contractor prior to use of the equipment. Upon request, the operator shall provide evidence of inspection and/or correction of any deficiency
CA: EHS advised the PIC that the gas-fired equipment inspection has not been available since 2023. An inspection by a qualified licensed contractor or local fire authority, if available, should be acquired as soon as possible
15 - ice from approved source; properly handled, supplied and stored; equipment approved/clean

EHS observed a black, mold-like substance on the interior door that prevents ice from escaping the ice machine, in the ice machines on floors 2, 3, and 4.
511-6-2-.07(8) - Ice Supply If self-service ice is provided, then it shall be from an approved water supply. In all new tourist accommodations, and in existing tourist accommodations when machines are replaced, only automatic dispensing ice machines will be allowed. In existing tourist accommodations permitted prior to the adoption of this rule, the use of existing self-service ice-storage bins may be continued, provided that the machines are maintained in good repair and capable of being properly cleaned and sanitized according to the manufacturer’s recommendations. Further, a copy of the manufacturer’s cleaning instructions shall be maintained onsite for review at the request of the Health Authority. Scoops, ice buckets, and other ice handling equipment, shall be of easily cleanable material and construction. They shall be stored in a clean place and shall be kept clean. Glassware shall not be used to scoop ice.
CA: EHS advised the PIC to ensure that interior contact surfaces of the ice machines are cleaned regularly
