- The original CFSM certificate shall be posted in public view in each food service establishment. An additional copy shall be retained on file at the food service establishment at all times, and shall be made available for inspection by the Health Authority.
Corrective action: PIC placed the CFSM documentation visible to the public at time of inspection.
12B - personal cleanliness
Regulation: 511-6-1.03(5)(j) - hair restraints (c)
Observed employee beard net and hat with no hair restraint for dreads reaching down lower back while handling food. Employee had bread net and hat.

- Employees preparing or handling food shall use effective and clean, disposable or easily cleanable nets or other hair restraints approved by the Health Authority, worn properly to restrain loose hair including beards and mustaches longer than one half inch.
Corrective action: Employee applied hair restrains to the dreads. Discussed that long hair is required to be controlled by hair restraints.

- The most current inspection report shall be prominently displayed in public view at all times, within fifteen feet of the front or primary public door and between five feet and seven feet from the floor and in an area where it can be read at a distance of one foot away.
Corrective action: Discussed with PIC to have most current inspection report posted to the public.

Observed grease build-up between the fryer and grill.

The food-contact surfaces of cooking equipment and pans shall be kept free of encrusted grease deposits and other soil accumulations.
- Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris.
Corrective action: Discussed with PIC to have areas cleared of build up.
