EHS observed dead lady bugs in the insect trap located by the back door that leads to the dumpster. Also EHS observed dead bugs in the lights in the ceiling.

Corrective action- PIC needs to make sure that any insects that are dead or alive need to be properly discarded.

511-6-1.07(5)(l) - Removing Dead or Trapped Birds, Insects, Rodents, and Other Pest (C) (l) Removing Dead or Trapped Birds, Insects, Rodents, and Other Pests. Dead or trapped birds, insects, rodents, and other pests shall be removed from control devices and the premises at a frequency that prevents their accumulation, decomposition, or the attraction of pests.

Violation: All physical facilities shall be maintained in good repair.The physical facilities shall be cleaned as often as necessary to keep them clean and by methods that prevent contamination of food products. Inspector note: EHS observed hood system not operating correctly in front service area and in need of cleaning. Corrective action: PIC will have hood system observed and fixed by maintenance.

Corrective action: EHS instructed PIC to have ice removed as frequently as to prevent such and/or to adjust temperature to prevent such.
