Observed visible present at the back door (door isn't flush with the frame). EHS informed the owner during the initial inspection. Per GM, the facility is still waiting on the seal/strip to be placed at the back door. (m) Outer Openings, Protected.
(i) Filling or closing holes and other gaps along floors, walls, and ceilings;
(ii) Closed, tight-fitting windows; and
(iii) Solid, self-closing, tight-fitting doors. EHS informed the GM to have the owner address the issue as soon as possible.

Observed facility's need to clean under equipment(drink station), clean ceilin ceiling tiles and light fixtures from dust accumulation. (b) Cleaning, Frequency and Restrictions.
The physical facilities shall be cleaned as often as necessary to keep them clean and by methods that prevent contamination of food products.
Except for cleaning that is necessary due to a spill or other accident, cleaning shall be done during periods when the least amount of food is exposed, such as after closing. EHS informed the GM of the need to routinely clean the facility.

Observed the facility needing to clean under equipment, change out stained ceiling tiles/ceiling duct vents/grates that are stained OR damaged. (5) Maintenance and Operation.
(a) Good Repair. All physical facilities shall be maintained in good repair.
(b) Cleaning, Frequency and Restrictions.
