Observed hot water temperature of room 102 at 124F.

511-6-2-.07(2) - Hot Water Supply Cold running water under pressure shall be provided to all equipment that uses water. Hot and cold running water under pressure shall be provided to all lavatories, bathing facilities, laundry facilities, and water-using equipment where eating and drinking utensils are washed. In all new tourist accommodations, and where possible in existing tourist accommodations, hot water in all guest rooms shall not exceed 120F.

Corrective action: Discussed with PIC to adjust hot water to not exceed 120F

Observed the following in each room:

114: Mattress cover stain 202: Mattress pad stained 301: Mattress teared at the bottom. 314: Pillow cover stain and mattress cover stain 408: Pillow cover stain 418: Pillow cover stain and mattress pad stain

511-6-2-.14(6) - Bed, Bedding, and Linens Washable mattress pads or covers shall be used on all mattresses. Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin. Each bed shall be provided, as a minimum, with two sheets and one pillow and pillowcase. After each occupancy and upon the request of the occupant, sheets and pillowcases shall be changed with fresh laundered linens. During occupancy, linens shall be changed at a minimum frequency of not less than weekly.

Corrective action: Discussed with PIC to have linen cleaned or replaced

Observed dumpster requiring repairs, part of lid is on the floor and the rest missing. Area around dumpster has broken wooden planks and broken glass bottles.

511-6-2-.14(5) - Condition of Furnishings All furniture, draperies, appliances, carpets, and other accessories, in the tourist accommodation, whether the property of the tourist accommodation owner or not, shall be considered the property of and furnished by the tourist accommodation for purposes of enforcement of this Chapter, and must be maintained in good repair, clean, and free of vermin.

Corrective action: Discussed with PIC, to have dumpster repaired and area cleaned

EHS observed the following in each room: Room 427- hair on sheets Room 425-hair on sheets and yellow stain on mattress pad Room 326-hair and stain on mattress pad Room 308-stain and hair on sheets and mattress pad. Room 304-stain on mattress pad Room 213-stain on mattress pad

CA-PIC needs to make sure that employees are changing each bed with clean bedding items. Any items that are stain and damaged must be replaced with new ones.

CA-PIC needs to make sure that housekeeping is changing each room with clean sheets and mattress pads. Any of the bedding items that are stained and/or damaged must be properly discarded and replaced with new ones.

EHS observed 5 out of 9 rooms with water exceeding 120 or below 100 degrees.

Corrective action-PIC needs to inform maintenance to adjust the water heater so that the water temperatures in each room ae between 100-120 degrees.

EHS requested gas-fire inspection report from PIC. PIC did not give EHS any records.

Corrective actions- PIC needs to get a annual gas and fire inspection completed for the facility. The document must be located at the facility.

511-6-2-.15(5) - Heating and Fire Safety-Inspection All gas-fired equipment shall be inspected at least annually by a qualified licensed contractor or local fire authority, if available. Points to be inspected are proper construction and installation, malfunctions and adjustments of controls and burners, faulty heat exchangers and vent obstructions. Any defects found on inspection must be corrected by a qualified heating contractor prior to use of the equipment. Upon request, the operator shall provide evidence of inspection and/or correction of any deficiency

EHS observed the following in each room:

Room 426- AC filters needs to be cleaned. Room 419- AC unit needs to be cleaned. The unit had a lot of dust. Room 418- AC unit needs to be cleaned. The unit had a lot of dust.

Corrective action- PIC needs to make sure that housekeeping are cleaning the air filters and the ac unit.

EHS observed the following in each room:

Room 106-Hair in the bathroom sink Room 429- hair in toilet. Room 424- Hair in shower and bathroom floor. Room 419- hair in shower. Room 418- hair in shower and bathroom floor. Room 408- hair in bathroom sink and hair in the shower.

Corrective action- PIC needs to ensure that housekeeping are cleaning each bathing facility. The bathroom should be free from debris and hair. Any hair or debris found must be properly cleaned and discarded.

511-6-2-.08(4) - Toilet Facilities-Cleaning and Maintenance Toilet rooms and fixtures must be smooth and nonabsorbent and shall be kept clean and in good repair. Walls, floors, and ceilings shall be constructed of easily cleanable nonabsorbent materials and shall be kept clean and in good repair. While being used by tourists, every surface of a bathtub, shower, shower enclosure, toilet and lavatory, which may come in contact with a person’s body, must be sanitized each day, unless the guest has declined regular tourist room services. If a tourist declines regular tourist room services, the tourist accommodation facility must ensure that these surfaces are cleaned and sanitized at least once per week and between tourists

EHS observed seven out of 9 rooms water exceeding 120. EHS will be discussing this with the PIC.

EHS observed trash and construction debris/waste surrounding dumpster in dumpster area. Violation: areas surrounding container shall be kept clean and orderly. CA: Remove construction debris/waste and any other trash around area; suggest to see if can get small roll-off dumpster or if can place construction debris/waste in dumpster/store elsewhere.

EHS observed dumpster missing lids. Violation: all outside refuse or garbage containers shall be provided with tight fitting lids or covers and shall be kept covered when stored. CA: Speak to waste management provider to see about replacing lids or dumpster. This must be corrected by August 2, 2023
