EHS observed side entrance near fitness center with gate that does not self latch. Violation: (h) Access gates shall also comply with the requirements of this Rule and shall be equipped to accommodate a locking device. Barrier gates shall open away from the pool in facilities constructed after adoption of this chapter. Pedestrian access gates shall be self-closing and have a self-latching device. Gates other than pedestrian access gates shall have a self-latching device. Where the release mechanism of the self latching device is located less than fifty-four inches from the bottom of the gate, 1. The release mechanism shall be located on the pool side of the gate at least three inches below the top of the gate and 2. Any openings in the gate or barrier located within eighteen inches of the release mechanism shall be less than one-half inch. 3. Mechanisms controlled by an access card reader, key entry device or keypad shall be located on the outside of the access gate. CA: Self closing hinge needs to be adjusted or replaced to allow for magnet to catch gate and keep close.

EHS observed flow meter with float stuck at bottom and unable to read flow. Violation: (b) A flow meter measuring the rate of flow through the filter system with an appropriate range readable in gallons per minute (GPM) and accurate within ten percent actual flow shall be provided. The flow indicator shall be capable of measuring from one-half to at least one and one-half times the design flow rate. The gauge shall be located after the filtering equipment and in such location on the return line, so as to measure the total amount of water returning to the pool according to the manufacturer’s installation specifications. CA: repair or replace flow meter so that flow can be read.

EHS observed no signs with peak occupancy and hours of operation near entrances. Violation: (c) A sign shall be posted stating the hours of operation in clear view near the entrance and shall state the theoretical peak occupancy CA: Ensure both peak occupancy and hours of operation are posted at all entrances.

EHS observed low alkalinity level at 57ppm. Violation: Alkalinity shall be between 60-180 ppm. CA: Adjust alkalinity to ensure it is between 60-180 ppm.

EHS observed low calcium hardness 90 ppm. Violation: Calcium hardness shall be between 150-1000 ppm. CA: Adjust calcium hardness to ensure it is between 150-1000ppm.

Observed high chlorine level and was unable to obtain a reading. c) Free Chlorine level in pools using cyanuric acid or a stabilized chlorine product; Minimum 2.0 Ideal 2.0-5.0 Maximum 10.0. EHS informed CPO who will address the issue.

Observed pressure gauge on 2nd filter needs to be adjusted as it isn't visible from its current location and isn't easily accessible. (a) A pressure or vacuum gauge or other means of indicating system condition shall be provided in the circulation system in an easily readable location. EHS informed CPO of the need to turn the pressure gauge so that it is visible.

Observed facility needing the required signage “WARNING - NO LIFE GUARD ON DUTY and RISK OF DROWNING - SUPERVISE CHILDREN CLOSELY” in 4 inche lettering.(b) Where no lifeguard is on duty, a sign or signs shall be placed in clear view at or near the entrance to the pool and shall state in clearly legible letters at least four inches high “WARNING - NO LIFE GUARD ON DUTY and RISK OF DROWNING - SUPERVISE CHILDREN CLOSELY”. EHS informed CPO of the need to purchase signage.

Observed pool hours and bather load (4 inch numbering) needing to be posted at or near all entry gates. (c) A sign shall be posted stating the hours of operation in clear view near the entrance and shall state the theoretical peak occupancy. EHS informed CPO of the need to post the noted information.

Observed pump with clear taped on the plumbing. EHS informed CPO to determine why the tape is on the plumbing and to ensure there are no issues with the pump or plumbing.

Observed low alkalinity level. CPO was aware and will address the issue.

Observed multiple entrance gates that are not self-closing or self-latching or missing latches. (h) Access gates shall also comply with the requirements of this Rule and shall be equipped to accommodate a locking device. Barrier gates shall open away from the pool in facilities constructed after adoption of this chapter. Pedestrian access gates shall be self-closing and have a self-latching device. Gates other than pedestrian access gates shall have a self-latching device. Where the release mechanism of the self latching device is located less than fifty-four inches from the bottom of the gate, 1. The release mechanism shall be located on the pool side of the gate at least three inches below the top of the gate and 2. Any openings in the gate or barrier located within eighteen inches of the release mechanism shall be less than one-half inch. 3. Mechanisms controlled by an access card reader, key entry device or keypad shall be located on the outside of the access gate. EHS informed PIC and property manager that the pool will have to close until all gates are operating effectively.

Observed little to no chlorine in the pool. c) Free Chlorine level in pools using cyanuric acid or a stabilized chlorine product; Minimum 2.0 Ideal 2.0-5.0 Maximum 10.0. PIC stated he hasn't had time to adjust the water chemistry.

Observed low PH levels in pool. b) During operation, the pH is less than the minimum or more than the maximum levels allowed in DPH Rule 511-3-5-.l7. PIC stated that he hasn't had time to adjust water chemistry.

Observed the skimmer baskets needing to be cleaned and water flow didn't seem the be producing enough flow in skimmers. PIC is to clean baskets and determine if skimmers are pulling accurately.

Observed CPO certificate not available on-site. (2) The trained operator shall have a current certificate showing completion of an approved operator training course. A copy or the original certificate or documentation shall be available onsite for inspection by the Health Authority. EHS informed PIC and management of the need to have a copy of the CPO certification on-site.

Observed low alkalinity levels. PIC stated he hasn't had a chance to balance the water chemistry.

EHS observed gate nearest fitness center not latching shut. Magnet closure not working. Chained shut currently will need to fix prior to permitting. Violation: Pedestrian access gates shall be self closing and self latching. CA: service device that controls magnetic closure.

EHS observed emergency phone box not functioning. No dial or ringing when button pushed. Violation: n operable, hard-wired, conventional telephone line or continuous powered source, weatherproof emergency phone shall be permanently installed in a conspicuous location within the pool enclosure and must be readily available to bathers at all times. The emergency phone shall be capable of connecting to 911 and electronically transferring an automatic number identification and automatic locator identification of the pool emergency phone to the public safety answering point, if available. A 911 sign or the names and phone numbers of the nearest available police, fire, ambulance service or rescue unit shall be posted nearby. CA: have device serviced.

EHS observed no throwing rope attached to ring buoys. Violation: A minimum one-fourth inch diameter throwing rope one and one-half times the maximum width of the pool or fifty feet in length, whichever is less, to which has been firmly attached a ring buoy with an outside diameter of approximately fifteen inches or a similar flotation device which is U.S. Coast Guard approved. CA: attach 1/4" diameter rope that is 1 1/2 times the maximum width of the pool in length or 50' whichever is less.

EHS observed flow rate of 100 gpm. Required flow rate for turnover rate of 6 hours is 135 gpm. Violation: The equipment for a swimming pool shall be of adequate size to turn over the entire pool water capacity. The turnover rate for pools constructed after the adoption of this chapter shall not exceed the sum of one and one-half times the average water depth in feet; where the number of hours is equal to the number feet calculated, or exceed once every six hours whichever is less. Unless the rate is otherwise specified in subsection (b) below. This system shall be designed to give the proper turnover rate based on the manufacturer's recommended maximum pressure flow of the filter in dirty media condition of the filter, immediately prior to cleaning the filter. CA: ensure filters and pumps are allowing for required flow rate.

EHS observed facility without CPO certificate onsite. Violation: The trained operator shall have a current certificate showing completion of an approved operator training course. A copy or the original certificate or documentation shall be available onsite for inspection by the Health Authority. CA: have copy available on site.

EHS observed hand rail nearest fire place is loose and missing flange cover. Handrail was concreted in but concrete has eroded and missing. Violation: Hand rails if removeable shall be installed to not be removed without tools. CA: replace flange plate. Or secure via concrete.

EHS observed "NO DIVING" written out on a single 4" tile. Letters shall be 4" tall. Universal international symbol for "No Diving" is not located on pool deck. Violation: The words "No Diving" and the universal international symbol for “NO DIVING” shall be permanently visible at the edge of the deck for water five feet in depth or less, placed only on the deck beside the depth markers, and shall conform to that outlined for depth markers in subsections (3)(a) thru (h) above. CA: place "NO DIVING" in 4" lettering and no diving symbol on pool deck.

Chapter 511-3-5-.06(3)(f). EHS observed missing hand rail for stairs leading from sundeck to pool on left side when looking from main office/club house. Violation: each set of stairs shall be provided with at least 1 hand rail. CA: replace missing hand rail.
