EHS observed damaged/dirty AC filters in AC units in all rooms observed.
Furnishings must be maintained in good repair, clean, and free of vermin.
EHS recommended to PIC to replace damaged AC filter cartridges and to clean out clogged/dirty units
Regulation: 511-6-2-.14(6) - bed, bedding, and linens
EHS observed damaged/thinning linens, stained sheets and stained pillowcases in all rooms inspected.
Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin.
EHS advised PIC to clean/replace damaged sheets. PIC stated that last linen shipment occurred in December of 2023 and that facility was expecting more

Regulation: 511-6-2-.08(5) - toilet and bathing ventilation
EHS observed in rooms 137 and 229 a mold-like substance on ceiling of bathrooms near showers, indicating lack of proper ventilation in bathrooms.
Violation Code Reference 511-6-2-.08(5) - Toilet and Bathing Ventilation Toilet, lavatory and bathing facilities shall be mechanically ventilated. Where ventilation ducts are used, ducts from toilet rooms shall not be connected into return ventilation ducts to any other room. In existing tourist accommodations permitted prior to the adoption of this rule, the current ventilation must prevent odor, mold, mildew, and moisture. All new or major renovation of a tourist accommodation will require mechanical ventilation to be installed to the outside of the building separate from heating and air-condition systems.

EHS observed lack of lighting in rooms 225 in both main room and bathroom.
