8 - annual gas fired equipment inspection on file; gas units equipped with automatic safety and/or 100% cut-off pilot
Regulation: 511-6-2-.15(5) - heating and fire safety-inspection
EHS observed no gas-inspection records on the premises. When asked, PIC was unable to proved records. PIC stated that he was new to the facility and unsure of when the last inspection was performed and who to contact for an inspection.
511-6-2-.15(5) - Heating and Fire Safety-Inspection All gas-fired equipment shall be inspected at least annually by a qualified licensed contractor or local fire authority, if available. Points to be inspected are proper construction and installation, malfunctions and adjustments of controls and burners, faulty heat exchangers and vent obstructions. Any defects found on inspection must be corrected by a qualified heating contractor prior to use of the equipment. Upon request, the operator shall provide evidence of inspection and/or correction of any deficiency
CA: Informed PIC of the annual gas-fired inspection requirement, local storage of inspection records, and advised to have an inspection performed as soon as possible, if no prior inspection can be found. Discussed that DPH inspection forms are available if needed
27 - all furniture, waste receptacles, draperies, carpets; clean, and in good repair, considered the property of and furnished by the tourist accommodation

511-6-2-.14(5) - Condition of Furnishings All furniture, draperies, appliances, carpets, and other accessories, in the tourist accommodation, whether the property of the tourist accommodation owner or not, shall be considered the property of and furnished by the tourist accommodation for purposes of enforcement of this Chapter, and must be maintained in good repair, clean, and free of vermin.
CA: Advised PIC to increase cleaning frequency of room furnishings and carpets
28 - beds, mattresses, mattress pads, springs, frames, pillows, and bed coverings clean; vermin free and in good condition; bed linen properly supplied, changed and laundered

EHS Observed:
- Stains on the mattress pads of rooms: 108, 207, 115, and 223
- Hairs on the sheets of rooms: 108, 115, and 207
- Apparent burn holes in the mattress pad of room: 115
- Stains on exterior of pillows of rooms: 223 and 207
511-6-2-.14(6) - Bed, Bedding, and Linens Washable mattress pads or covers shall be used on all mattresses. Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin. Each bed shall be provided, as a minimum, with two sheets and one pillow and pillowcase. After each occupancy and upon the request of the occupant, sheets and pillowcases shall be changed with fresh laundered linens. During occupancy, linens shall be changed at a minimum frequency of not less than weekly.
CA: Advised PIC to clean and replace linen with stains and/or hairs. Advised PIC to dispose of and replace linen with burn holes

511-6-2-.08(4) - Toilet Facilities-Cleaning and Maintenance Toilet rooms and fixtures must be smooth and nonabsorbent and shall be kept clean and in good repair. Walls, floors, and ceilings shall be kept clean and in good repair.
