EHS observed
-can opener with encrusted food debris on make table. Per PIC, can opener is used.
-ice spout in front food service with brown/green slime.
Equipment food-contact surfaces and utensils shall be clean to sight and touch.
PIC removed items, placed into wash basin.
10D - food properly labeled; original container
Regulation: 511-6-1.04(4)(d) - food storage containers identified with common name of food (c)
EHS observed bin of unidentified white powder in dry storage unlabeled.
Except for containers holding food that can be readily and unmistakably recognized, such as dry pasta, working containers holding food or food ingredients that are removed from their original packages for use in the food establishment, such as cooking oils, flour, herbs, potato flakes, salt, spices, and sugar shall be clearly and legibly identified, in English, with the common name of the food.

EHS observed that permit was not posted.
Post the permit in a location in the food service establishment that is conspicuous to consumers.

15A - food and nonfood-contact surfaces cleanable, properly designed, constructed, and used
Regulation: 511-6-1.05(6)(a) - good repair & proper adjustment (c)
EHS observed:
-grease hood nonfunctional, displaying warning lights re: filter system -sanitizer mixer not drawing sanitizer into bin; facility manually mixing sanitizer.
Equipment shall be maintained in a state of repair

15C - nonfood-contact surfaces clean
Regulation: 511-6-1.05(7)(a)2,3 - equipment, food/nonfood-contact surfaces, and utensils, food-contact surfaces of cooking equipment & nonfood-contact surfaces free of accumulations (c)
EHS observed buildup of grease on:
-exterior front, side flashing of grease hood -exterior top/side of prep top cooler adjacent to grill -exterior front, sides of grill area
Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris.

17C - physical facilities installed, maintained, and clean
Regulation: 511-6-1.07(5)(a),(b) - good repair, physical facilities maintained; cleaning, frequency & restrictions, cleaned often enough to keep them clean (c)
EHS observes buildup of excess grease from ventilation hood building up on ceiling fixtures above grill sufficient to cause dripping.
The physical facilities shall be cleaned as often as necessary to keep them clean and by methods that prevent contamination of food products.
PIC to clean, maintain area.
17D - adequate ventilation and lighting; designated areas used
Regulation: 511-6-1.05(3)(d) - ventilation hood system, adequacy, adequate to prevent grease & condensation build-up (c)
EHS observes nonfunctional grease hood system. Per PIC, electric system is damaged. Warning lights are on.
Ventilation hood systems and devices shall be sufficient in number and capacity to prevent grease or condensation from collecting on walls and ceilings.
PIC to repair or replace grease hood.
