8 - annual gas fired equipment inspection on file; gas units equipped with automatic safety and/or 100% cut-off pilot
Regulation: 511-6-2-.15(5) - heating and fire safety-inspection
EHS requested to view the facility's gas-fired equipment inspection. The PIC provided an inspection report performed on 02/01/2024 by Watson Plumbing. The PIC indicated that a company has been contacted to perform a new inspection, and are scheduled to perform the inspection on Thursday, January 15.
511-6-2-.15(5) - Heating and Fire Safety-Inspection All gas-fired equipment shall be inspected at least annually by a qualified licensed contractor or local fire authority, if available. Points to be inspected are proper construction and installation, malfunctions and adjustments of controls and burners, faulty heat exchangers and vent obstructions. Any defects found on inspection must be corrected by a qualified heating contractor prior to use of the equipment. Upon request, the operator shall provide evidence of inspection and/or correction of any deficiency.
CA: EHS advised the PIC that gas-fired equipment is to be inspected annually by a qualified licensed contractor or local fire authority, if available, and that an inspection is to be acquired as soon as possible
28 - beds, mattresses, mattress pads, springs, frames, pillows, and bed coverings clean; vermin free and in good condition; bed linen properly supplied, changed and laundered

EHS observed the following in each room:
- Room 103 - Hairs on the mattress pad, sheets, and mattress cover; stains on the mattress pad, pillow, and mattress cover.
- Room 121 - Stains on mattress cover and mattress pad; hairs on the mattress cover.
- Room 227 - An apparent burn hole in the comforter; stains on the mattress cover and pillow.
- Room 217 - Tears in the mattress cover.
- Room 302 - Stains on the mattress pad and sheets.
- Room 304 - Stains on the mattress cover and mattress pad
511-6-2-.14(6) - Bed, Bedding, and Linens Washable mattress pads or covers shall be used on all mattresses. Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin. Each bed shall be provided, as a minimum, with two sheets and one pillow and pillowcase. After each occupancy and upon the request of the occupant, sheets and pillowcases shall be changed with fresh laundered linens. During occupancy, linens shall be changed at a minimum frequency of not less than weekly.
CA: EHS advised the PIC that all linen items are to be clean and in good condition, without stains and/or holes. The PIC is to ensure that housekeeping is inspecting the linen items—removing stained or damaged items from use. Linen with stains should either be re-cleaned or discarded and replaced. Linen with holes/tears should either be repaired or discarded and replaced
