Observed quat sanitizer in sani buckets (x2) measuring 0 ppm when using test strips to verify concentration. Per PIC, facility was previously having issues with sanitizer dispenser; however, maintenance had arrived to repair this morning. Had PIC to flush the lines and remake sani buckets. Buckets rechecked and were at appropriate concentration.

Observed old dried on sauce on the interior of the round portion utensil that is used to portion items on pizzas stored as clean above the 3-C sink. Per PIC, facility no longer uses these portion pizza rounds. Explained that all food contact surfaces shall be cleaned to sight and touch. PIC removed and placed in the 3-C sink to be rewashed.

Observed an employee jacket and bookbag stored on top of boxes of pizza sauce near the stand up white freezer in the main kitchen area. All employee personal items shall be stored in a designated area to prevent contamination of food, equipment, utensils, and/or single service items. Employee removed to appropriate location.
Observed a pair of jeans stored on an empty drink bottle crate in the main kitchen near the warming cabinets. All employee personal items shall be stored in a designated area to prevent contamination of food, equipment, utensils, and/or single service items. Facility is to move.

Observed an employee's McDonald's bag stored in the warming cabinet closest to the computer desk in the main kitchen area. Per PIC, facility staff are not allowed to store personal food products in these units. All employee personal items are to be stored in a designated area to prevent contamination of food, equipment, utensils, and/or single service items. PIC discarded.

Violation: 1. Utensils shall be maintained in a state of repair or condition that complies with the requirements specified under subsections (1) and (2) of this Rule or shall be discarded. Inspector note: EHS observed containers in WIC storing marinara sauce cracked. Corrective action: PIC will have the containers discarded and replaced.

Violation: 1. Dressing rooms or dressing areas shall be designated and used if employees routinely change their clothes in the establishment. 2. Lockers or other suitable facilities shall be provided and used for the orderly storage of employees' clothing and other possessions. Inspector note: EHS observed facility lacking employee designated area in WIC for drinks and in RIF for drinks as well. Corrective action: PIC will implement employee designated area.
