Observed several foods stored in the walk in freezer that were not maintained as frozen. Food temperatures are remaining at 41F or below. CTM, Facility shall repair walk in freezer and send invoice to austin.haney@dph.ga.gov within 72 hours.

Observed food items under condensation leak in the walk in freezer. COS, PIC moved food/shielded food for protection from contamination.

Observed rodent droppings in the dry storage area in the main kitchen. Facility is aware of the issue and is currently working with Astro exterminators. Facility will update me at austin.haney@dph.ga.gov with reports from the exterminators.

Observed only handwashing sink in the main kitchen with no paper towels. COS, PIC replaced the paper towels in the dispenser.

Observed food items stored under condensation leak in the walk in freezer. COS, PIC stated that the frezzer had been repaired recently so the new one is having issues and moved the food out from under the condensation leak to prevent contamination.

Observed several employee drinks not in a single service container with a lid and straw in the main kitchen. COS, PIC removed the cups from the kitchen and instructed employees on when/how to drink in the kitchen.

Observed no paper towels at the handwash sink in the main kitchen. COS, PIC replaced the paper towels.

Observed several chemical buckets and a chemical sprayer stored in the handwash sink. COS, PIC removed the items from the handwash sink and instructed employees that the handwash sink is for handwashing only.

Observed a large build up of mold in the ice dispenser in the dining area. COS, PIC discontinued use of the ice machine until the cleaning company can come clean/replace it.
