8 - annual gas fired equipment inspection on file; gas units equipped with automatic safety and/or 100% cut-off pilot
Regulation: 511-6-2-.15(5) - heating and fire safety-inspection
Observed no documentation available to show evidence that annual gas-fired equipment has been annually inspected. CA: If annual inspection has not been completed or has expired, inspection must be performed and report submitted to health department. Updated inspection report must be available for review in the facility
27 - all furniture, waste receptacles, draperies, carpets; clean, and in good repair, considered the property of and furnished by the tourist accommodation
Regulation: 511-6-2-.14(5) - condition of furnishings
Observed the following: Stains on pullout couch and dirty AC filters in room 211. Cabinet door unattached with exposed screw, stained couch in room 209. Burns in the ottoman and stains and trash left on the pullout couch in room 210. Long hairs on the room phone, broken drawers, stains and trash left on pullout couch in room 307. Torn curtains and excessive build up on AC filters in room 404. CA: All furniture, draperies, appliances, carpers and other accessories in the tourist accommodation must be maintained in good repair and kept clean. Corrections must be made before rooms can be rented out
28 - beds, mattresses, mattress pads, springs, frames, pillows, and bed coverings clean; vermin free and in good condition; bed linen properly supplied, changed and laundered

Observed stained mattress and box spring in room 220, stains on mattress protector in rooms 211 and 404, debris on bedding in room 404. CA: Beds, mattresses, mattress pads and all coverings shall be clean and free of vermin. Corrections must be made prior to renting rooms
34 - grounds, walkways, porches and hallways maintained; drives provide unobstructed width and proper spacing; graded to drain; trailer spaces marked clearly, areas clean and maintained in good repair

Observed lights not working or turned off in both emergency stairwells. Observed missing bulbs in the hallway on the fourth floor. Observed flooring in disrepair in the elevator. CA: Common areas and hallways shall be maintained and kept in good repair. Informed PIC of repairs needed
16 - toilet/lavatory/bathing facility fixtures and rooms clean and maintained; properly lighted, clearly marked, proper signage, and all in good repair

Observed shower head with build up and in need of cleaning in room 217, hole in the bathroom wall in room 209, debris in the sink and tub in room 307, toothpaste in the sink and build up in the basin of the toilet in room 404. CA: Toilet rooms and fixtures shall be kept clean and in good repair. Corrections will be made before rooms are rented out
24 - openings to outside and physical facilities protected by tight-fitting doors, screening, or other means

Observed exterior side door propped open. CA: Openings to the outside shall be effectively protected against the entrance of rodents and insects by tight-fitting, closed doors. Informed PIC
25 - floors, walls, ceilings, windows, doors clean, and in good repair; appropriate for location and operation

Observed exposed wires coming from the wall in room 217, floor with dirt/debris on the floor and in need of vacuuming in room 220, torn carpet in room 211, fraying carpet in room 210, hair clip from previous guest on floor in room 404. CA: Floors, walls, ceilings, windows and doors shall be properly maintained in good repair and shall be kept clean. Corrections must be made before rooms are rented
