4-1A - food separated and protected
Regulation: 511-6-1.04(4)(c)1(i)(ii)(iii)(v)(vi)(vii)(viii) - packaged & unpackaged food separation, packaging, and segregation (p, c)
Observed raw pork and raw ground beef stored on same tray in RIC. CA: All raw meats and RTE foods shall be stored according to minimum internal cook temp. COS: PIC placed raw pork and raw beef on separate trays in bottom of RIC.
2-2B - proper eating, tasting, drinking, or tobacco use
Regulation: 511-6-1.03(5)(k)1&2 - eating, drinking, or using tobacco (c)
Observed employee drinking a bottled smoothie drink during inspection. CA: Employee drinks shall be in a single service disposable cup with a lid and a straw.
2-2D - adequate handwashing facilities supplied & accessible
Regulation: 511-6-1.07(3)(b) - hand drying provision (pf)
Observed hand sink with no hand drying provision. CA: Hand sinks must be fully stocked at all times. COS: PIC placed a roll of paper towels at hand sink.
15C - nonfood-contact surfaces clean
Regulation: 511-6-1.05(7)(a)2,3 - equipment, food/nonfood-contact surfaces, and utensils, food-contact surfaces of cooking equipment & nonfood-contact surfaces free of accumulations (c)
Observed exteriors of coolers and freezer not clean to sight or touch, with buildup of accumulations, floors, walls, and ceilings with build up of grease and dust, microwave not cleaned at a frequency to prevent accumulations. CA: Equipment, floors, walls, and ceilings shall be clean and free of accumulations.
17C - physical facilities installed, maintained, and clean
Regulation: 511-6-1.07(5)(a),(b) - good repair, physical facilities maintained; cleaning, frequency & restrictions, cleaned often enough to keep them clean (c)
Observed areas of kitchen with broken floor tiles, missing ceiling tiles over 3 compartment and meat sink, baseboards missing portions exposing the unsealed wood beneath, open AC vents with missing covers, ceiling tiles with evidence of water damage. CA: Physical facilities must be maintained in good repair.
17D - adequate ventilation and lighting; designated areas used
Regulation: 511-6-1.07(4)(b) - designated areas for employee activity, located to prevent contamination of food, equipment, utensils, linens, & single service articles (c)
Observed employee watch stored on shelving above clean dishes, employee personal items stored above facility equipment. CA: All employee personal items shall be stored in order to prevent potential contamination of facility equipment or food. COS: PIC relocated personal items.
