Observed TCS food (dressings, butter, sour cream) improperly cold holding above 41 degrees F (see temp log). CA: PIC stated cooler has not been working and food items had been in reach in cooler for less than 4 hours, moved items to WIC to rapidly cool.

Observed recently prepped salads cooling by unapproved methods; salads were tightly sealed, stacked on top of one another, and placed in a working reach-in cooler. Temperatures of other TCS foods in the same unit had raised above 41 degrees F. CA: PIC relocated salads to the walk-in cooler to rapidly cool. All TCS foods placed in working reach-in coolers should be at 41F or lower. Discussed proper methods of cooling.

Observed several employees handling food without wearing a hair restraint (hat, hairnet, visor). CA: Discussed with PIC. Hair restraints were put on.

Observed potentially hazardous TCS foods (sour cream, salsa, assorted salad dressings, milk) improperly cold holding above 41 degrees Fahrenheit in reach-in coolers. CA: Discussed with PIC. Items that were out of temp for less than 4 hours were moved to the walk-in cooler to rapidly cool. Items out of temp for more than 4 hours were discarded.
