Basic - Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Small ice machine.

Basic - Equipment in poor repair. Observed multiple Low Boy doors broken/off hinge, multiple Low boy drawers not closing properly and multiple make table and low boy cooling units inoperable. Observed all cold held foods held on ice excluding low boy drawer under griddle. Previously noted 03/11/25

Basic - Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Observed accumulation of dust on ceiling tile and vent between make table and chest freezer.

Basic - Employee beverage container in a cold holding unit with food to be served to customers. Observed employee beverage stored on shelf in walk-in cooler.

Basic - Floor area(s) covered with standing water. Observed standing water on floor at entrance leading to walk-in cooler nearest office.

Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed black mild-like substance on shelving located in walk-in cooler nearest office.

Basic - Wall soiled with accumulated grease, food debris, and/or dust- under hood vents.

Basic - Ceiling/ceiling tiles/vents soiled with accumulated dust, above protein make table.

Basic - Wall soiled with accumulated grease, food debris, and/or dust. Accumulation of grease dripping from hood vent down wall behind grill.

Basic - Dishmachine not washing/rinsing properly. Must wash, rinse and sanitize all dishware, equipment and utensils in three-compartment sink until dishmachine is functioning properly. Observed dish machine sanitize reading 0ppm chlorine with test strip. Operator properly set up triple sink, sanitize sink reading 200ppm quat with test strip. Corrective Action Taken

Basic - Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Observed cell phone stored on prep table beside employee engaged in active prep.

Basic - Employee wearing jewelry other than a plain ring on their hands/arms while preparing food. Observed employee engaged in prep with Watch on left wrist.

Basic - Equipment in poor repair. Observed low boy cooler door broken from hinge and several low boy coolers and drawers inoperable.

Basic - Hole in or other damage to wall, in warewash area.

Basic - No Heimlich maneuver/choking sign posted. Operator printed and properly posted during inspection. Corrected On-Site
