High Priority - Single-use gloves not changed as needed after changing tasks or when damaged or soiled. Employee picked up an item off the floor with gloves on and then engaged in making a frozen custard. I informed the manager and he had the employee to remove the glove wash his hands and put on a new pair of gloves. Corrective Action Taken

Basic - Accumulation of debris on exterior of warewashing machine. Bottom of the dishwasher doors has a white build up.

Basic - Bathroom facility in disrepair. Sink is off the wall in the handicap stall in the ladies bathroom. The door is locked.

Basic - Floor soiled/has accumulation of debris. Debris build up on the floor on the side of the beverage syrup rack in the dry storage area.
