Observed toilet not flushing in the Ladies restroom, not operating properly at time of complaint inspection General Manager notify Regional Manager at time of inspection, maintenance notify on Sunday per General Manager, complaint inspection on Tuesday 9/1/2025. Recommend to place sign out of order on Ladies restroom. Corrective Action Taken

Basic - Frozen time/temperature control for safety food being slacked at room temperature is no longer frozen solid. Observed steak and rotisserie unthawing at room temperature spoke to Store Manager, proper method of thawing. Manager removed immediately. Corrected On-Site

Basic - Reach-in cooler interior/shelves have accumulation of soil residues. Observed mod like growth on the bottom of reach in cooler locate in the back prep area.
