Basic - Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Upon inspection observed air conditioning unit vents on ceiling with debris build up in kitchen.

Basic - Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Upon inspection observed cell phone on top of cutting board by hot holding station in kitchen. Operator removed cell phone during inspection. Corrected On-Site

Basic - Hole in or other damage to wall. Upon inspection observed multiple areas with wall in disrepair. Repeat Violation

Basic - Ceiling/ceiling tile shows damage or is in disrepair. Upon inspection observed leak from ceiling tile in back hallway by water heater.

Basic - Exterior door has a gap at the threshold that opens to the outside. Side door leading to outside with gap present. Repeat Violation

Basic - Hole in or other damage to wall. Behind water heater wall exposed and bottom of walk in freezer with rust present damage to bottom metal plate. Repeat Violation

Basic - Ice buildup in walk-in freezer. Repeat Violation

Basic - Stored food not covered. Upon inspection observed small pan of sausage patties close door door of walk in freezer. Operator corrected storage during inspection. Corrected On-Site

Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime. Soda dispenser with debris build up by dispenser nozzles.

Intermediate - Non-pitting surface rust on food-contact equipment. Dry storage rack right side of side door entrance with rust present.

Basic - Stored food not covered. Upon inspection observed case of donut holes uncovered in walk in freezer. Operator corrected storage during inspection. Corrected On-Site

Basic - Wall soiled with accumulated grease, food debris, and/or dust. Under 3 compartment sink.

Basic - Water leaking from pipe and/or faucet/handle. Under 3 compartment sink with leaking pipe present. Repeat Violation
