Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime: observed the flat surface around the nozzles of the soda machine in the drive thru area are dirty. General Manager had an employee clean during the inspection. Corrected On-Site

High Priority - - From initial inspection : High Priority - Raw sewage on ground of establishment. Observed lift station overflowing and emptying into storm drain. Admin Complaint - From follow-up inspection 2025-05-22: At time of the callback inspection. Repairs underway on the lift station pumps. Lift station pumped out during the callback. No sewage observed overflowing. Okaloosa County Health Department updated. D6 DM notified. All interior drains and restrooms checked to ensure no backups. Admin Complaint - From follow-up inspection 2025-05-23: At time of the callback Lift station repairs ongoing. No sewage overflow observed. Area dry . Admin Complaint - From follow-up inspection 2025-06-06: Lift station has been repaired and is functioning correctly. There is no raw sewage on the ground in the area. None of the drains in the establishment are backing up into the establishment. Admin Complaint

High Priority - Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet: Mop sink has a splitter on it with only one hose. There is a vacuum breaker on the top of the splitter but not on each side. Inspector explained to the General Manager the correct positioning of the vacuum breaker and the need for a second one if they keep the splitter.

High Priority - Employee failed to wash hands before putting on gloves to initiate a task working with food. - employed picked a piece of food up off the floor with his gloved hand. He removed the glove as through the piece of food away. He then grabbed another glove to put on without washing his hands. Inspector gave corrective feedback on proper procedures. Corrective Action Taken

Intermediate - No proof of required state approved employee training provided for employee hired more than 60 days ago. To order approved program food safety material, call DBPR contracted provider: Florida Restaurant and Lodging Association (SafeStaff) 866-372-7233. - establishment has eight employees on duty, other than the manager, and there are only two employee food safety training cards. Inspector explained to manger on duty concerning the need for training.

Basic - Current Hotel and Restaurant license not displayed. - establishment's license is current but Assistant General Manager could not locate.
