High Priority - Live, small flying insects. Observed approximately 8 live flying insects flying around and over the preparation table and pizza dough, preparation table located near the walk in cooler. Observed approximately 30 live insects flying all around the mop sink. Observed 3+ live insects flying around the three compartment sink in the kitchen. Observed around 2 live insects flying at front counter area by the office area located in the kitchen. Also observed 1 live maggot crawling on the floor by exit door.
