High Priority - Observed employee touching cell phone and Bluetooth device then proceeded to put on gloves to prep order without washing hands first. Advised manager and he instructed employee to remove gloves and wash hands. Corrected On-Site

Basic - Duct tape used to repair prep table and wall in kitchen area. Repeat Violation

Basic - Ice buildup in pizza make table. Repeat Violation

Basic - Observed metal pizza shovel stored on floor between uses. Manager moved off floor. Corrected On-Site

High Priority - Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed garlic and oil mix held at 61F - Cold Holding. Per manager mix is kept at ambient temperature and had only been out for about 1 hour. Manager placed in ice bath to rapid cooling. Mix was located on prep table.
