Basic - Ice buildup in reach-in freezer and/or walk-in freezer. Observed ice build up on the floor of the walk-in-freezer

High Priority - Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Observed "Y" connector at mop sink with missing vacuum breaker on the non-chemical side of the "Y" connector.

Intermediate - No paper towels or mechanical hand drying device provided at handwash sink. Observed no soap or towels at hand wash sink in kitchen. Manager advised that they had run out. Supply truck arrived during the inspection and manager restocked hand wash sinks with soap and paper towels. Corrected On-Site

Basic - Single-service articles not stored inverted or protected from contamination. Observed coffee filters in dry goods storage area not inverted or protected.

Basic - Wet mop not stored in a manner to allow the mop to dry. Observed wet mop stored in a manner not allowed to dry.

High Priority - Food-contact surfaces in contact with time/temperature control for safety food not cleaned at least every four hours. Observed donut display case in front of cans counter with old food and debris. Also nozzles of hot chocolate machines and espresso machine heavily soiled.

Basic - Bowl or other container with no handle used to dispense food. Observed large drink cup stored in sugar bin. Manager removed during inspection.

Basic - Cardboard used to line food-contact shelves. Observed soiled card board used as lining on top shelves of two reach in coolers. One reach in cooler located near front counter, other located near kitchen .

Basic - Floor not cleaned when the least amount of food is exposed. Floors near and underneath ovens heavily spoiled with debris from old food.

Basic - Ice buildup inside inside walls of dairy dispensing units. One located near front counter, other located in prep area of kitchen.

Basic - Ice scoop handle in contact with ice. Observed entire ice scoop touching ice in bin underneath ice coffee counter. Employee removed during inspection. Also observed metal ice scoop placed in soiled counter in between uses.Corrected On-Site Corrected On-Site

Basic - No conspicuously located ambient air temperature thermometer in holding unit. Observed no ambient thermometer in reach in coolers where milk is stored. One near front counter, one near prep table in kitchen.

Basic - Single-service articles improperly stored. Observed two cases of single service cups on floor in storage unit. Manager removed during inspection. Repeat Violation

Basic - Working containers of food removed from original container not identified by common name. Observed two white bins of a white powdered substance on a cart near prep table. Employee stated that two white bins ware sugar. Manager labeled during inspection. Corrected On-Site
