Basic - Ice buildup in reach-in freezer and/or walk-in freezer. Observed ice build up behind cooling unit and on floor in walk-in-freezer Repeat Violation

High Priority - Time/temperature control for safety food identified in the written procedure as a food held using time as a public health control has no time marking and the time removed from temperature control cannot be determined. Observed liens on the line; cut tomato and cut leafy greens lacking time marks. Adjusted sticker time marks were added. Corrected On-Site

High Priority - Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Observed lack of back flow device on the non chemical side of the splitter in the mop sink.

Basic - Bathroom facility not clean. Observed the men,s restroom floor has an accumulation of paper. ALSO, the floor has an accumulation of water, (and 2 wet floor signs).

Basic - Carbon dioxide/helium tanks not adequately secured. Observed the bulk CO2 tank is not secured.

Basic - Floor area(s) covered with standing water. Observed standing water on the floor by the bulk CO2 tank.

Basic - Hood filter missing from automatic fire suppression/exhaust system. Observed all hood filters over the broiler are missing. Filters were replaced. Corrected On-Site

Basic - Ice buildup in reach-in freezer and/or walk-in freezer. Observed the the insulated pipes behind the walk-in freezer fans have an accumulation of ice. The freezer door was ajar(leading into the walk-in cooler). The freezer door was then closed.

Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed the internal hood system has an accumulation of grease(filters are not in place).

Basic - Worn, torn and/or soiled floors/carpeting. Observed the floor under the dining area soda machine, is soiled. Repeat Violation

Basic - Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Ice chutes on soda machine in lobby have slime and mold build up. Employees cleaned area. Corrected On-Site

Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Slime and debris at beverage drink station on outside nozzles on machine as well as counter area underneath single service containers for straws, lids, etc. Employees cleaned area. Corrected On-Site

Basic - Floor soiled/has accumulation of debris. Observed floor in walk-in freezer soiled with old food debris.
