Observed employee use restroom then return to work without washing his hands. Spoke with operator. Corrective Action Taken

Basic - Buildup of food debris/soil residue on equipment door handles. Observed equipment door handles on stand up reach in cooler in the fry station with food debris.

Basic - Floor soiled/has accumulation of debris. Observed floor in walk in freezer soiled.

Basic - In-use utensil in non-time/temperature control for safety food not stored with handle above top of food within a closed container. Observed pitcher handle in contact with sugar in the bulk use bin next to the ice machine on the front line.
