Observed employee use restroom then return to work without washing his hands. Spoke with operator. Corrective Action Taken

Basic - Buildup of food debris/soil residue on equipment door handles. Observed equipment door handles on stand up reach in cooler in the fry station with food debris.

Basic - Floor soiled/has accumulation of debris. Observed floor in walk in freezer soiled.

Basic - In-use utensil in non-time/temperature control for safety food not stored with handle above top of food within a closed container. Observed pitcher handle in contact with sugar in the bulk use bin next to the ice machine on the front line.

Basic - Backflow device not located for convenient service or maintenance access. Observed no backflow preventer on mop sink.

Basic - Buildup of food debris/soil residue on equipment door handles. Observed door handles on stand up cooler like in the fryer/ chicken breading area soiled with food residue.

Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed buildup of food debris on walk in cooler shelves, and black like substance on reach in cooler door gaskets on the front service line near tea dispensers.

Basic - Reach-in cooler shelves with rust that has pitted the surface. Observed rust on reach in cooler shelving at the front counter area near pick up window.

High Priority - Single-use gloves not changed as needed after changing tasks or when damaged or soiled. Observed employee leave salad station to place salad n reach in cooler, no glove change or hand washing observed upon returning to station, educated employee on proper glove usage. Corrective Action Taken
