Intermediate - Employee applied hand antiseptic in place of washing hands as required. Observed employee videoing him making a drink for a customer while serving the customer and engage in food prep and didn't wash his hands.

Basic - Equipment in poor repair. Gasket around the walk in cooler door and the employee only door is missing its door handle.

Basic - Hole in or other damage to wall. Observed all walls area in the lobby is in disrepair,showing of rooted wall from water damage. Observed outside wall on the parking lot area and front of the building has structural damage with the stucco.
