Basic - Hood filters missing from automatic fire suppression/exhaust system. Filters on the floor of kitchen.

Intermediate - Spray bottle containing toxic substance not labeled. Spray bottle unlabeled at bar. Operator labeled container.

Basic - Wet mop not stored in a manner to allow the mop to dry. Mop in bucket in hallway leading back into bar.

Intermediate - Certified Food Manager or person in charge lacks knowledge of employee health policy and health requirements. Spoke with operator about Big 6 and reporting responsibilities. Send health packet to print for kitchen.

Intermediate - Raw/undercooked animal food offered and establishment has no written consumer advisory. Raw animal foods must be fully cooked prior to service. Printed out consumer advisory for operator to hang and they hung it up at the bar. Corrected On-Site

Basic - Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Black substance on inside of ice bin.

Basic - Fifty (50) foot-candles of light not provided at a food working surface or where safety is a factor. No light over grills and fryers had to use flashlight to look at surfaces.
