Basic - Food-contact surface of silverware provided for customers at salad bar/buffet not properly protected. Observed fork in dining room not wrapped or presented food contact surface down. Operator discarded and replaced with wrapped plastic ware. Warning

Basic - No handwashing sign provided at a hand sink used by food employees. Observed a handwashing sign missing from the men's restroom. Warning

Intermediate - No proof provided that food employees are informed of their responsibility to report to the person in charge information about their health and activities related to foodborne illnesses. Emailed form and discussed importance of reviewing and having all team members sign. Corrective Action Taken Repeat Violation

Basic - Wall soiled with accumulated grease, food debris, and/or dust. Observed walls soiled by walk in freezer, fuse panel wall, and throughout perimeter of kitchen.

Basic - Wet mop not stored in a manner to allow the mop to dry. Observed wet mop stored in mop bucket of dirty water.

Marked exit/path to marked exit blocked. For reporting purposes only. Observed card board boxes in path of fire exit door, not broken down or flattened.

Intermediate - Establishment has no written procedures for employees to follow in response to a vomiting or diarrheal event where the vomit or diarrhea is discharged onto surfaces in the establishment. Emailed and printed informational hand out. Corrected On-Site

Basic - Floor tiles missing and/or in disrepair. Observed broken floor tile in kitchen and by walk in cooler.
