Basic - Buildup of food debris/soil residue on equipment door handles. Upon inspection observed multiple equipment handles soiled with old food debris on main cook line.

Basic - Equipment and utensils not properly air-dried - wet nesting. Upon inspection observed moisture between stacked pans on drying racks.

Basic - Ice buildup in reach-in freezer and/or walk-in freezer. Upon inspection observed ice buildup inside walk-in freezer.

Basic - Equipment and utensils not properly air-dried - wet nesting. Upon inspection observed moisture between stacked pans on drying rack. Manager proceeded to separate pans for drying at time of inspection. Corrected On-Site

Basic - Wet mop not stored in a manner to allow the mop to dry.

Basic - Floor soiled/has accumulation of debris. Floor in back area of kitchen has accumulation of grease.

Basic - Carbon dioxide/helium tanks not adequately secured. Upon inspection observed 2 carbon dioxide tanks not secured in the back of the kitchen.

Basic - Ceiling/ceiling tile shows damage or is in disrepair. Upon inspection observed ceiling throughout kitchen damaged and in disrepair, mold like substance on top of both walk-in cooler and walk in freezer.

Basic - Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Upon inspection observed multiple air conditioning vents in disrepair and soiled with mold like substance throughout kitchen.

Basic - Floor area(s) covered with standing water. Upon inspection observed multiple areas in kitchen with standing water.

Basic - Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Upon inspection observed air conditioning vents rusted throughout restaurant.

Basic - Ice buildup in reach-in freezer and/or walk-in freezer. Upon inspection observed ice buildup inside walk-in freezer.

Basic - Wet mop not stored in a manner to allow the mop to dry.

Intermediate - No paper towels or mechanical hand drying device provided at handwash sink.

Basic - Food stored on floor. Upon inspection observed chicken tenders stored on floor in walk-in freezer. Upon inspection observed 2 boxes of lemons stored on the floor in walk-in cooler.

Basic - Single-service articles improperly stored. Upon inspection observed to go boxes stored on floor in the kitchen by make line.

Basic - Ceiling/ceiling tile shows damage or is in disrepair. Upon inspection observed ceiling paint peeling throughout kitchen area.

Basic - Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Upon inspection observed dining room vents soiled with mold like substance.

Basic - Floor area(s) covered with standing water. Upon inspection observed standing water by 3 sink compartment in kitchen. Repeat Violation

Basic - Floor tiles missing and/or in disrepair. Upon inspection observed tiles missing and broken throughout kitchen. Repeat Violation

Basic - Utensils in poor condition. Upon inspection observed reach in cooler handle in disrepair. Repeat Violation

Basic - Floor area(s) covered with standing water. Standing water on kitchen floor below steamer/3 Compartment Sink. Repeat Violation

Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime. Clean equipment wire rack across from 3 compartment sink with food debris buildup present.

Intermediate - Multiuse food-contact surface not smooth, free of breaks/cracks/chips/pits/crevices. Upon inspection observed plastic food containers in disrepair.

Basic - Ceiling vents missing. Multiple in kitchen.

Basic - Ceiling/ceiling tile shows damage or is in disrepair. Dining room ceiling tiles with what appears to be water damaged present

Basic - Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Multiple in kitchen and multiple in dining room.

Basic - Equipment and utensils not properly air-dried - wet nesting. Upon inspection observed small stack pans and small trays with moisture in between, across from 3 compartment sink.

Basic - Equipment in poor repair. Reach in cooler handles in disrepair in cook line area and door gasket in disrepair in left side of cook line. Ice machine exterior in disrepair by cashier.

Basic - Floor area(s) covered with standing water. Left side of 3 compartment sink under the steamer on the floor , water present.

Basic - Garbage on the ground and/or pad around dumpster.

Basic - Interior of refrigerator or freezer in disrepair/has exposed insulation. Walking cooler door with insulation exposed and inside bottom right corner in disrepair.

Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Upon inspection observed hot holding equipment with food debris build up bottom door gasket to reach in freezer left side of cook line with debris present and base of can opener with debris present. Walk-in cooler fan covers with debris present. Outdoor receptacle with grease present.

Basic - Open dumpster lid.

Basic - Single-service articles improperly stored. Upon inspection observed case of disposable cups stored on ground in front of walk-in freezer.

Basic - Standing water in bottom of reach-in-cooler. Reaching cooler across from fryers in kitchen area.

Basic - Storage of maintenance equipment/tools in areas that may result in cross contamination of food, equipment, utensils, linens and/or single-service and single-use articles. Upon inspection observed hammer and mallet on shelf next to single service items across from 3 compartment sink.

Basic - Walk in cooler shelves with rust that has pitted the surface.

Basic - Wet mop not stored in a manner to allow the mop to dry. Upon inspection observed wet mop on the floor in dry storage area.

Basic - Food stored on floor. In the walk in freezer box's stored on the floor.

Basic - Worn, torn and/or soiled floors/carpeting. Floor underneath the fryers has grease built up. Grout throughout the kitchen in disrepair.
