Basic - Bathroom located inside establishment not completely enclosed with tight-fitting, self-closing doors. Men's restroom door not closing tightly creating a gap. Manager states door is on scheduled to be repaired. Corrective Action Taken

High Priority - Toxic substance/chemical improperly stored. Case of degreaser cleaner stored on top of single service lids in outside storage shed. Manager moved chemical to proper storage. Corrected On-Site

Basic - Ceiling/ceiling tiles/vents/walls/floors soiled with accumulated food debris, grease, dust, or mold-like substance. Floor under fryers on right side soiled. Area on both sides of fryer with food debris.

Basic - Garbage can located outside has no lid or lid open/broken. Dumpster has right side lid broken off and laying on the ground. Left lid in disrepair.

Intermediate - No soap provided at handwash sink. No soap available for both restrooms. Operator installed soap dispensers during inspection. Corrected On-Site
