Intermediate - Handwash sink used for purposes other than handwashing. Hand sink in back kitchen area next to three compartment sink contained food debris peppers sink used as dump sink.

Intermediate - No soap provided at handwash sink. Hand sink in back kitchen area had no soap and dispenser broken.

High Priority - Stop Sale issued due to food not being in a wholesome, sound condition. Observed sliced tomatoes in walk in cooler with unidentified debris. See stop sale.

Basic - Toilet/urinal not flushing/functioning properly. Women's room out of order due to toilet not flushing properly. Per operator plumber to arrive today, 9/12/25, to correct issue. Warning

High Priority - Roach activity present as evidenced by live roaches found. Observed 6 live roaches in sticky trap under sandwich make tables in front kitchen area. Warning

Basic - Dead roaches on premises. Observed 2 dead roaches in back storage area behind soda fountain boxes in corner of kitchen area. Warning
