Intermediate - No paper towels or mechanical hand drying device provided at handwash sink. Observed no paper towels at handwash sink in rear of kitchen. Operator replaced paper towels. Corrected On-Site

Intermediate - Soap dispenser at handwash sink not working/unable to dispense soap. Observed hand sink at rear of kitchen with soap dispenser unable to dispense soap. Operator replaced batteries and soap dispenser is able to provide soap. Corrected On-Site

Basic - Ice buildup in reach-in freezer and/or walk-in freezer. Observed ice build up in lowboy cooler at front counter.

Basic - Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Observed employee personal drink, glasses, bible and hand sanitizer stored on food contact tables at front line and by ice machine. Operator removed items. Corrected On-Site
