High Priority - Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed raw salmon (51F - Cold Holding) inside flip top cooler at cook line. Per operator, food not prepped or portioned today; food stored out of temperature for approximately 2 hours. Operator put food inside reach in freezer to quick chill. Food cooled to 41F during inspection. Corrected On-Site

High Priority - Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Observed no vacuum breaker on splitter at mop sink.
