Observed wall tiles broken/ missing behind rotisserie oven at front line. 2.) Floor area(s) covered with standing water. Observed water pooling in rear kitchen area where tiles are cracked and in mop sink. 3.) Wall soiled with accumulated grease, food debris, and/or dust. Observed at wall above triple sink and wall behind drying rack for pans and cooking utensils. 4.) Ceiling/ceiling tile shows damage or is in disrepair. Observed ceiling tiles with water damage in hallway leading to bathrooms. Repeat violation. 5.) Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Observed throughout kitchen, dining room and bathrooms. Repeat violation.

Operator stated work order in for repairs. Corrective action taken. 4.) Cove molding at floor/wall juncture broken/missing. Observed at ware washing area. Observed several floor tiles damaged in hallway leading to Protein cooler and in mop sink. Observed water pooling in some of these areas. Operator stated work order in for repairs. Corrective action taken.
