Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime. Observed mold like substance in ice machine. Repeat Violation

Intermediate - Handwash sink used for purposes other than handwashing. Observed blue rags stored in hand wash sink next to display donuts.
