High Priority - Live, small flying insects found Observed one flying insect near fryer at front line and one flying insect near walk-in-cooler.

High Priority - Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Observed Vacuum breaker missing from non chemical side of splitter at mop sink. There is a vacuum breaker installed above the splitter. Discussed with operator.

Intermediate - Records/documents for required employee training do not contain all of the required information. Observed multiple food safety certifications missing DOB info. Operator stated she will get those up to date.

Basic - Equipment in poor repair. Observed gasket on walk-in-cooler torn.

Basic - Interior of oven/microwave has accumulation of black substance/grease/food debris. Observed interior of microwave in prep area soiled. Also, interior of oven has debris. Repeat Violation

High Priority - Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. In the steam tables on the cook line, meatballs (110/72F - Hot Holding). Operator had employee remove and reheat. At conclusion of inspection reheated to correct temperature. Corrected On-Site

High Priority - Toxic substance/chemical improperly stored. Chemical spray bottle of peroxide on prep room table. Operator relocated the chemical spray bottle. Corrected On-Site

Basic - Clean glasses, cups, bowls, plates, pots and pans not stored inverted or in a protected manner. Blue ice bucket on top of bulk ice bin not inverted or covered.

Basic - Equipment and utensils not properly air-dried - wet nesting. On drying rack and shelf by three compartment sink, clean plastic containers stacked while wet.

Basic - Floor soiled/has accumulation of debris. Walk in cooler floor has build up of food debris.

Basic - Interior of oven/microwave has accumulation of black substance/grease/food debris. Microwave on prep line with food debris residue build up.
